Candidates applying to Lacordaire Academy are required to submit to the admissions committee:

Completed Application Form
Birth/Baptismal Certificate
Application fee of $75

The sending school must forward:
Health records and record of immunizations
Recent standardized test scores
Recent report card
Teacher Recommendation
